gladysGladys Morris, Administrative Assistant & Office Manager, will answer your questions about services, fees, insurance, billing, and appointmentavailability.

She will be happy to take your insurance information and have your sessions pre-certified before your first appointment so that you will know what your out of pocket expenses should be.

She will take your inquiry calls and make your appointments.

Charges, Insurance, and Appointment Times Available

See the Individual Clinician’s Pages for information on their charges, insurance panel participation, and availability of appointment times/days.

Clients are responsible for all charges.

Full payment is expected at the time of service.

Payment is expected at the time of service (unless previously arranged) for other clinicians in these offices.

Insurance Billing and Coverage

If we are billing your insurance company, we will file your claims (once per claim) as a courtesy to you.  You are responsible for any charges that your insurance does not pay (including deductibles, co-pays, not covered charges, out of pocket charges, and any other additional charges).  You are responsible for understanding the contents, requirements, and limits of your coverage and will be expected to make informed decisions about seeking and continuing services that may not be covered by your insurance.

Please have your insurance benefits pre-certified by Gladys before your first appointment.

We will need a copy of your insurance card and your driver’s license.

Before Your First Appointment

Peggy Ferguson, Ph.D., Linda Evans, Ph.D., and Lexie Smith, LPC have individual paperwork that they would like for you to fill out and bring with you when you come for your first appointment.

Maintaining Active Status

In order to maintain active patient status, patients are expected to regularly schedule and keep appointments as medically necessary and determined by the practice. Although, inactive charts will be closed, patients who wish to return to services may have their charts reopened.

Payment

Cash, personal checks, or credit cards are accepted for payment at the time of service.  You may also make payments for sessions online on this website with a credit card.

Cancellation Policy

If you do not show up for your scheduled appointment, and you have not notified us at least 24 hours in advance, you will be required to pay a $75.00 late cancellation fee that is not billed to an insurance company.

Offices are closed on Fridays, holidays, and weekends.

Bad Weather

People come from all over to work with this group of clinicians.  If school is cancelled in your locale because of bad weather, the late cancellation fee is waived for not coming in.

We want you to be safe and not have a wreck trying to get to us.  In case of bad weather please call and leave a voice mail or an email to Gladys or to your clinician. saying that you won’t be here.

Emergency Information

The clinicians in this office are not available on a 24 hour per day, seven days a week basis.  Patients treated in these offices are deemed appropriate for this level of care during the admission and assessment sessions.

In case of emergency, please present to your local hospital emergency room.

Consultation With Other Clinicians

All of the clinicians in our offices are in private practices.  If you would like for your clinicians to consult with other clinicians within these offices, or with other clinicians (including your medical doctor) you must fill out and sign a “Consent to Release Information”, which specifies the clinicians to consult, and the information to be released.
